Sildenafil: Commonly known by its brand name, Viagra, Sildenafil is one of the most widely used oral medications for ED. It works by inhibiting an enzyme called PDE5, which relaxes the blood vessels in the penis and improves blood flow. This effect helps achieve and maintain an erection during sexual stimulation. Sildenafil typically starts working within 30-60 minutes and lasts for about 4-6 hours. It’s most effective when taken on an empty stomach.
Tadalafil: Sold under the brand name Cialis, Tadalafil is known for its longer-lasting effects, often referred to as the “weekend pill.” Like Sildenafil, it also inhibits PDE5 and increases blood flow to the penis, but its effects can last up to 36 hours. This longer window allows for more spontaneity in intimate activities. It can be taken daily in smaller doses or as needed in higher doses.
Trimix: This is an injectable treatment combining three active ingredients: alprostadil, papaverine, and phentolamine. Together, they work to dilate blood vessels and relax smooth muscle tissue in the penis, leading to an erection. Trimix is often recommended for men who do not respond to oral medications. It requires proper training for self-administration and precise dosing to minimize side effects, such as prolonged erections (priapism).
PT-141: Also known as Bremelanotide, PT-141 is a synthetic peptide that works by activating receptors in the brain related to sexual arousal. Unlike PDE5 inhibitors that focus on improving blood flow, PT-141 targets the central nervous system to enhance desire and arousal. It is usually administered as a subcutaneous injection and can be effective for both men and women experiencing sexual dysfunction.
